New patients generally require more time than established patients as we get to know you, evaluate your needs, and discuss your medical history. Please be prepared to complete a medical history questionnaire that will ask about your medical conditions, surgeries, medications and treatments.
For your convenience, the medical history questionnaire is also available on our website for you to complete prior to your visit.
Please bring your current insurance card and a picture identification card for all visits. Our receptionists will scan a copy of each into your electronic medical record.
Your health insurance plan may require that you obtain a referral from your primary care physician (PCP) prior to your first visit.
